I have completely fell off the wagon. I have been to the gym probably twice in the last 3 weeks. I need help getting my mind to re-focus . Any ideas out there to help me do that?
242.4 lb Lost so far: 40.6 lb.    Still to go: 112.4 lb.    Diet followed N/A.
gaining 1.2 lb a week

Take it minute by minute. Make a healthy choice when selecting something to eat and tell yourself that you are worth eating healthy and living a long life. You can do it, you can. Just take the first step, whatever that is for you.
