Yesterday I discussed that my YMCA has switched to the CDC materials for their Diabetes Prevention Program. I did a little research and found this link. If you drill down, you can even find the program handouts for each lesson. However, this program works best when you are enrolled in an official course and have the support of your coach and fellow students.
