I was weighed at my doctor's on Wednesday- I am 4lbs lighter at the surgery than I am at home and that is with my clothes on.... so do I record what I weigh first thing in the morning on my scales or should I deduct 6lbs taking into account what I weighed at the doctors fully clothed- not getting new scales!
167.0 lb Lost so far: 0 lb.    Still to go: 13.0 lb.    Diet followed reasonably well.

if you are tracking yourself on your scale then just use those numbers. makes things too complicated to keep subtracting the difference or if you have a model that allows adjustments you can adjust it down.
