I think we need more info about your diet, health and lifestyle to offer useful feedback. Not sure I could help, but surely someone would see themselves in your description and can share what they did that worked for them.

some of your food log entries states some of what you ate, do you forget to log some of your food?? Looking over what I see, you don't have much if any veggies, just higher carb foods. I would introduce more lower carb veggies and try to keep your protein more stable. a more balanced food plan I think would help you.

Might ask your doctor for a consultation with a dietitian. I don’t think that this site is an appropriate place to ask for advice. Ask a professional how to develop a plan you can keep to for the rest of your life.
