"a considerable percentage of people will not be able to reliably enter nutritional ketosis eating in the range of 50-60 net carbs per day-" What's your point anyway reapeating that? I never said that is a permanent level. It is a level for a few days one give body time to accomodate for the fuel change.

Volek doesn't know everything. Many big people go in light ketosis already around 50-60 g/day. That has be confirmed by lab tests.

clinical experience has shown that holding dietary carbohydrate at 20-25 grams per day is often necessary...a few months later, following substantial weight loss, some individuals "might" be able to increase daily carbohydrate intake above 60 grams per day. " Also "nutritional ketosis is by definition a benign metabolic state that gives human metabolism the flexibility to deal with famine or major shifts in available dietary fuels." It's not that strict. It works even if you have some days with low 20-25 g/level and some other days a bit more (keep it under 80-100 g/day though and one should get to back to a state 'carb-saturated') Just make sure you have more ketosis days than days without it. Getting in ketosis is what resets the insulin sensitivity in lever and somewhat in muscles. Though muscles may need certain type exercise, too and take longer time to reset. Silly if Volek has said one must follow some carb rules strictly to obtain results. The ketosis and calorie restriction are the keys to take home here, not how one achieves them.
