I don't know if you have your intake calendar set on private or if you are not writing anything down but step #1 is writing it down, then if someone could see it they might help. I was probably in the same boat as you a couple years ago. I weighted 230 lbs. I got down to 180 in a year with the help of a weight loss professional and weight watchers. Then I screwed it up by going off on my own and slipped back into old habits. Yes, I blamed stress, being over worked, over tired, but actually all the work and being over tired came from my nutrition. That is where the doctor came in.
She told me don't even worry about exercise until you get the diet under control. Then you will start to feel better and will actually feel like you have more energy and want to exercise. She was right.
