Just bought several pairs of size 34 waist pants and a new belt to replace the size 44's I was wearing last December before starting my weight loss program. Loss of 10" inches around the waist. That's where the "Bad Fat" is supposed to be stored, Heh, heh, heh. Gotta be some health benefits there :-] Nice to not have a huge wad of clothes gathered around my waist that I had to cinch down with a belt with 10 extra holes. Much more comfy not having to keep pulling my pants up all of the time too. Especially when trying to walk with both hands full. LOL.

Been tweaking my Excel program for calculating dual wave boluses on my Medtronics 722 pump. I have the program automatically calculating my insulin to carb ratio based on the time of day using the computer clock. I have 3 ICRs (Insulin to Carb Ratios) during the day.

It also figures in the amount of carb grams to add if my Blood glucose is below my target of 84 mg/dl and subtracts the insulin for those carbs from my net meal carbs. If my BG is higher than 84 it also calculates a corrective bolus based on my insulin sensitivity factor (ISF). I have two factors. 20 for when my BG is above 130 and it is between midnight and noon. This covers the added insulin resistance that the Dawn Phenomenon induces. The rest of the time I use an ISF of 30.

I can also enter the amount of active insulin that is still in my blood from a previous bolus (based on my pump readout). This helps prevent insulin stacking.

I then enter the net carbs of my meal and the grams of either the fiber or the protein I'm eating. Not both. I've found that for me, it works better this way. If its a high fiber meal (more fiber than protein grams), I enter the fiber grams and the opposite if protein is the majority. The calculator takes 40% of the fiber or protein and adds it to the net carbs to give me the grams of "Total Effective Glucose" for the meal. The calculator then uses all of this info to compute the Dual Bolus Total, Now Bolus, Square Bolus, and duration of square bolus and displays them in 4 boxes at the bottom of the sheet. It uses the Net carbs and any adjustments due to BG corrections or active insulin and time of day to compute the Now Bolus part. The Square part of the bolus is computed using the protein or fiber effective glucose computation. The duration of the square bolus is computed using a sliding scale based on my current BG. Lower BG = less insulin delivered per hour. Higher BG = more insulin/hour. I then use this info to enter a dual wave bolus into my pump.

The format of the calculator results boxes is exactly the same as my pump input pages, so setting up a dual bolus is very easy and quick. Also, everything is very precise and no vaiable is omitted (AFAICT). It is producing very good results for me and my after meal spikes and hypos are virtually none existant using this method. All of the variables can be tweaked infinately if I need to make adjustment in the future.
