I started Keto early last year and just kept losing and gaining the same 3-4 lbs. I couldn't figure out what I was doing wrong and after 3 months, I got frustrated and quit. I am in my mid 50's and menopausal and it is definitely more difficult. But after going from 193 in March 2017 to about 210 in December I knew I had to find a way. In June 2018 I decided to try Keto again. I read researched and became very honest with myself. If I ate to my macros but then had a handful of nuts (or, lets be real, a few handfuls of nuts) I am no longer sticking to my macros. Also, the whole fat adapted thing never happened. I was eating more protein than Fat and I was always hungry so there was no calorie deficit. Now, I have temporarily given up nuts, because I can't have a handful. I count everything I eat and get enough fat. I am not always hungry and most days have a deficit. In the past 5 weeks, I have gone from 206 to 191.6. I will keep it going.
192.0 lb Lost so far: 0 lb.    Still to go: 42.0 lb.    Diet followed N/A.

Yeah, cals matter (for some ppl anyway) and that fat macro has to be high and those carbs have got to be low. If you are starting out stick to counting whole carbs m. It will help. Also I went threw a period of super slow loss and and then 2 weeks of no loss with very strict/ perfect & religiously counted cals/macros. Sometimes the body just doesn’t want to let go. But it has to Eventually!

Depends on what kind of nuts you are eating. The breakdown of carbs/protein/fats is different depending on the nut.

yes keeping balance is so important, I did the same thing for a long time, never kept track of macro's or calories and once I studied and learned to do it correctly everything changed. congrats on your work.
